The card is available on Registration basis for all patients.

Card validity is for one year effective from the date of registration and can be a renewed unless the medrich care management decides to change the care terms and conditions.

Medrich Care Management reserves the right to modify or cancel the privileges at any time the without prior notice.

Patient is responsible for providing and updating the correct personal information. In case the card is missing the member should notify the Medical center front office staff and request for a new card. Patients are required to present the EID/Passport on each visit to the Medical center together with the privilege card.
